Maxine from Addaction emailed me over the Summer regarding some work she had for me, she wanted a praise and concern form putting together for staff and users of the Addaction services to use. This meant it needed to be clear and concise and easy for the audience to navigate through the form and fill in the details. The original was in Comic Sans with Microsoft Clip Art - basically awful. Basically, this form needed to be completely scrapped and started again.
Due to my gemma@gembobs.co.uk account being deleted without my knowing so, I cannot access the emails to show the original attachment. Here is my version however, its not fantastic, but its clear and uses the company colours, which is what was asked of me.
